How to Switch Your Medicaid MCO to PA Health & Wellness
and Choose A Daughter’s Hand Home Care
(Community HealthChoices – CHC)

 

 

This guide explains, step by step, how individuals enrolled in Pennsylvania’s Community HealthChoices (CHC) program can change their Medicaid Managed Care Organization (MCO) to PA Health & Wellness and select A Daughter’s Hand Home Care as their provider for Home and Community-Based Personal Assistance Services.

Step 1: Confirm You Are Enrolled in Community HealthChoices (CHC)

Community HealthChoices (CHC) is Pennsylvania’s Medicaid program for individuals who are age 21 or older and need long-term services and supports. This includes older adults and individuals with physical disabilities.

You can confirm your CHC enrollment by checking your ACCESS card or by contacting Pennsylvania Enrollment Services.

Step 2: Contact Pennsylvania Enrollment Services

To change your Medicaid Managed Care Organization (MCO), call:

Phone: 1-800-440-3989

Tell the representative:
“I would like to change my Community HealthChoices MCO to PA Health & Wellness.”

The representative will verify your information and let you know when your change will take effect. Most changes begin on the first day of the next month.

Step 3: Select PA Health & Wellness as Your CHC MCO

Once PA Health & Wellness is selected, they will manage your Medicaid benefits under the Community HealthChoices program, including personal assistance services provided in your home and community.

Step 4: Choose A Daughter’s Hand Home Care as Your Provider

After your MCO change is completed (or while it is processing), notify your CHC Service Coordinator.

Tell your Service Coordinator:
“I would like A Daughter’s Hand Home Care to be my provider for Personal Assistance Services.”

Your Service Coordinator will update your service plan and send the referral to A Daughter’s Hand Home Care.

Step 5: Begin Services with A Daughter’s Hand Home Care

Once the referral is received, A Daughter’s Hand Home Care will contact you to:
- Complete intake paperwork
- Review your approved service hours
- Match you with a caregiver
- Schedule the start of services

Important Information for Individuals with Medicare and Medicaid

If you have both Medicare and Medicaid (also called dual eligibility):

- Changing your Medicaid MCO to PA Health & Wellness will NOT affect your Medicare
- Your Medicare doctors, hospitals, and prescription drug coverage stay the same
- Medicare and Medicaid are separate programs

You are only changing who manages your Medicaid benefits under CHC.
